POSITION SUMMARY:
Case Manager for Family Stability Program  

Reports to:         Manager, Clinical Services
Full/Part Time:    Full Time
Status:                Hourly/Non-Exempt  
Work Location:   Collington Square Elementary School (primarily)

What you will do

As the Case Manager, you will be an essential team member responsible for ensuring individuals and families enrolled in the Family Stability Program meet the program goals and objectives. Case management is a combination of project management and administrative functions all in support of the success of families with school-aged children living in the 21205 and 21213 zip codes. You will provide full program support to a caseload of up to thirty (30) individuals/families. This role involves ongoing assistance, following initial monetary aid to prevent eviction or utility turn-off.  

As the Family Stability Program Case Manager, you will:
  • Screen referrals
  • Conduct an intake
  • Devise a plan to support the family AND
  • Assess how we can best assist with their financial need
You will be instrumental in supporting three (3) core areas of work: 
  1. Prevention of homelessness
  2. Disrupt student mobility
  3. And increase self-sufficiency
While you will report to the Clinical Manager, you can also anticipate inquiries from the Grants Manager and/or the funder, United Way of Central Maryland. You may have prep meetings with both the Clinical Manager and Grants Manager prior to quarterly meetings with, and the annual site visit from, the funder.
 
You will also collaborate with, and assist, the Community School Coordinator (CSC) with building and maintaining strategic partnerships for workforce development, adult education, financial literacy; planning large- and small-scale community events; drafting memos and correspondence; fundraising research, grant writing and reporting. The CSC may be at Collington Square, and/or from other schools within the 21205 and 21213 zip codes.
 
Tasks and Responsibilities 

  • Promote the Family Stability Program and enroll our South Baltimore Adult High School students and/or those older youth enrolled in our workforce development programs.
  • Attend and participate in community-based events to inform residents of the program
  • Support the completion of required paperwork and secure documentation as needed to ensure the financial commitment to prevent eviction and utility turn-off notices.
  • Assess program participants to identify appropriate supports (financial, transportation, court involvement, child support and/or other related assessments)
  • Maintain detailed case notes in data collection software (Apricot 360)
  • Utilize best practices methods from the social work profession including a client-centered approach
  • Provide financial, workforce and continuing education coaching support
  • Provide retention and follow-up services and supports to program participants 
  • Cross promote Elev8 Baltimore programs (e.g. Adult High School, Extended Learning, Leadership Development, etc.) 
  • Participate in school-level team meetings aligned with the community school strategy
  • Attend required meetings with Elev8 and United Way 
  • Draft and edit memos, correspondence and outreach communications
  • Perform other job-related duties, as assigned
Communication and Documentation 
  • Complete all required paperwork in a timely manner 
  • Maintain all required documentation 
  • Review and provide support to maintain accurate and timely documentation in Elev8 systems 
  • Prepare reports for program, partners and funding sources 
Agency/Team Cohesion 
  • Responsible for attending mandatory departmental meetings 
  • Attend agency meetings and events 
  • Complete required training on time 
  • Maintain compliance with agency policies and procedures 
  • Prepare for and attend regularly scheduled meetings with supervisor 
  • Uphold organization values related to race, equity, and liberation of people and communities of color 

QUALIFICATIONS:
  • Bachelor’s degree preferred
  • Passionate commitment to improving communities of color, urban public schools, and to the mission of Elev8 Baltimore
  • Professional, with 2-4 years of experience providing case management within education, youth development, workforce development and/or community development programs
  • Excellent project management, problem-solving and analytical skills 
  • Ability to develop and maintain effective and positive working relationships with a broad range of stakeholders including parents, educators, community residents, elected officials, funders, and the business community
  • Ability to learn quickly and succeed in the rapidly changing environment
  • Ability to produce high-quality work and great volume in a mission-driven, results-oriented culture, and to respond appropriately in pressure situations with a calm and even temperament
  • Dependability, initiative, and ability to follow-through on assignments
  • Proficient in the use of Zoom, as well as Microsoft Office software - including PowerPoint software to prepare presentations for the Elev8 Baltimore team
  • Proven organizational skills with the ability to handle multiple tasks, track details and plan and act proactively 
  • Strong verbal and written communication skills
  • Strong interpersonal skills and ability to work independently and as a team
  • Must be able to deliver disappointing news/information with diplomacy, tact, and kindness
  • Ability to function with minimal supervision and manage multiple tasks

Compensation & Benefits
The hourly rate is $23.08 with a comprehensive benefits package available. Full benefits are offered including health insurance (medical, dental, and/or vision), paid vacation and sick time, retirement plan, mileage, and support for professional development opportunities. 

Work Environment/Location 
You will primarily work out of Collington Square Elementary/Middle School and will maintain regular office hours in the school to support ease of access for the community.  

Work Hours
Core Work Hours are 8:30 am – 5:30 pm. The Initiative hours can begin as early as 7:00 am and run as late as 7:00 pm. Pre-planned activities may include weekend and evening hours. There may be occasional emergency occurrences where extended hours to support a child and their family. This position requires traveling to homes or neutral locations to meet with families. Own transportation recommend and reliable transportation is required.
